How to Enroll in Before and After School Programs

2022-2023 Before/After care program is currently full. If you'd like to be added to a waitlist please email info@smartfitkidsacademy.com 

 

1. Please fill out the registration form found here and submit it online. 

2. Download the following forms from Registration-->Child Care Forms and get them signed by the doctor and email them to info@smartfitkidsacademy.com or drop them off to the 780 Ritchie Highway, Suite 26-28, Severna Park, MD21146

 - Emergency Form ( OCC1214)
 - Health Inventory (OCC1215)

 - Immunization form and record
 - Maryland Department of Health and Mental Hygiene Blood Lead Testing Certificate ( DHMH Form 4620)
