We are thrilled to welcome you and your child to Smart Fit Kids! Thank you for choosing our program; we are

 excited to  support your child's growth and provide a fun and enriching after-school experience.

 

Please remember that all forms are required before your child(ren) can be confirmed for our after school

​

Please ensure you have submitted the following forms :

-          Transportation form

-          Recurring Payment form

-          Emergency forms, please make sure to fill out at least one emergency contact

-          Health Inventory part I and part II ( signed by the doctor)

-          Immunization record

-          Blood lead Testing

-          Medication administration and Action Plans if applicable
