​Medication Forms
If your child needs to take medication during camp, or has other pertinent health concerns, please fill out the required forms and email them to camp@smartfitkidsacademy.com at least 2 weeks prior to the start of camp. Missing forms may affect your child's enrollment.

Part I and II of the Medication Administration Form, or any required action plans, must be dated, signed, and stamped by a physician. All forms must be completed in their entirety to be accepted.
If you are unable to submit the medication form, you must complete the No Medication Form, stating that you will not be providing medication administration forms or medication for that diagnosis. If your child requires medication that has not been supplied, you will be required to pick them up within 10–20 minutes, or emergency services (911) will be called.

We have numerous trained staff for medication administration, basic first aid, and CPR administration. We also have a camp nurse on call who is able to assist our staff.
Medication Administration Forms
Medical Administration Form (MMA-A/B)
This form is required for any medications to be administered to campers while at camp. Please use version A for 1-3 medications, and version B for more than 3 medications.
Allergy and Anaphylaxis Medication Administration and Authorization Plan
This form is required for campers with allergies and acts as an authorization for staff to administer allergy and anaphylaxis medicine, such as an EpiPen, in the case of an allergic reaction.
Asthma Medication Administration Authorization Form
This form is required for campers with asthma and acts as an authorization for staff to administer asthma-related medication, such as an inhaler.
Seizure Action Plan Form
This form is required for campers who may experience seizures while at camp, or who have been diagnosed with a seizure disorder.
No Medication Form
This form is required for campers with any diagnosis who will not be supplied medication, and acts as an acknowledgement that staff cannot provide or administer any medication to the camper.
Please note: Forms must be within date of expiration at all times while your child is at camp. Forms past the expiration must be immediately replaced with a newly completed form. Any expired medical forms that are necessary for your child will affect your child's enrollment.
If a parent is unable to complete a form before it expires (e.g. unable to schedule a doctors appointment), they may complete a No Medication Form until a new medication form is completed and returned to SmartFit Kids.

Only forms that are signed, dated, and stamped by a physician are considered complete. Uncomplete forms can not be accepted. All forms must be completed in their entirety to be accepted.

Our camp staff certified to administer medicine will administer only medicine provided by the parent/guardian. Camp staff are not permitted to supply medication to campers, including sunscreen or bug spray.
