School Year Forms
Please complete all forms as soon as possible to finalize your child's enrollment. Please review forms in ProCare and ensure all forms will be non-expired for the upcoming school year. Expired forms at any point are not accepted. All medical forms last one year.
We have numerous trained staff for medication administration, basic first aid, and CPR administration.

Transportation Permission Form
Permission form for our staff to transport your child between your child's school and our center.
Field Trip Permission Form
Permission form for our staff to transport your child on camp days and and for after school field trips.

Our enrollment contract for the school year.
Photo and Video Release
Permission for SmartFit Kids to take and use your child's photo or video.
Payment Authorization
Authorization for SmartFit Kids to setup automatic recurring payments for the school year.
Medical Forms and Plans
**The following forms (4) are required to be completed for every student enrolled.
Medical action plans are completed on a need-basis for every student.
Emergency contact**
MSDE Emergency Form, which may require a signature from a health provider.
If your child DOES have a medical condition (e.g. ADHD, asthma, allergies, etc.), please fill out the second page, complete with a provider's signature and stamp.
If your child does NOT have a medical condition, please fill out the name and DOB on page 2 and write N/A in each section.
Health Inventory**
MSDE Health Inventory form, which requires an signature by a health care provider.
Immunization Record**
MSDE Immunization Certificate, which requires an signature by a health care provider.
Blood Lead Test**
MSDE Blood Lead Testing Certificate, which requires an signature by a health care provider.
Allergy and Anaphylaxis Plan
This form is required for students with allergies and acts as an authorization for staff to administer allergy and anaphylaxis medicine, such as an EpiPen, in the case of an allergic reaction. This form requires an signature by a health care provider.
Asthma Action Plan
This form is required for students with asthma and acts as an authorization for staff to administer asthma-related medication, such as an inhaler. This form requires an signature by a health care provider.
Seizure Action Plan
This form is required for students who may experience seizures while at camp, or who have been diagnosed with a seizure disorder. This form requires an signature by a health care provider.
No Medication Form
This form is required for students with any diagnosis who will not be supplied medication, and acts as an acknowledgement that staff cannot provide or administer any medication to the student.
